Colorado Medicaid terminated providers by month
Colorado HCPF publishes an Excel list of providers whose Health First Colorado (Medicaid) participation was terminated for cause, with a separate list of reinstated providers.
Built from the Terminated Provider List edition Exclia retrieved on October 15, 2026, which has 321 entries. 15 of them carry an action date between October 2025 and September 2026.

HCPF calls it a termination list, not an exclusion list. A provider terminated for cause may not receive Medicaid reimbursement; the reasons are defined as “good cause” in 10 CCR 2505-10, Section 8.076.1.7.
About 320 rows. Each row has the provider name, a “doing business as” name, the NPI, the termination authority and the termination effective date. Some rows have no NPI.
Reinstated providers are removed from the termination list and appear on the separate Reinstated Provider List, with both the termination and reinstatement dates.
Each edition has its effective date in the file name (for example, “ef. 9.1.26”), so open the files from the HCPF page rather than a saved link.
Each figure counts entries in the current edition of the state’s official list by the action date the state publishes for each entry: the exclusion, suspension or termination date, depending on the list. Entries with no date are counted in the list size but not in any month.
A state list shows what the state publishes today. When a state removes an entry, for example after reinstatement, it leaves every month it was counted in, and a new edition can add an entry with an earlier date. The figures describe the edition named on the page.
States publish on their own schedules and some lists cover terminations or suspensions rather than exclusions, so counts are not comparable across states as rates. No names or identifiers are published here. To check a name, use the free exclusion check.
