Exclia

Delaware Medicaid exclusions by month

Delaware publishes a multi-page PDF report of provider actions from several authorities, including reinstatement dates, so each row needs reading in full.

Built from the Sanctioned Provider Report edition Exclia retrieved on October 8, 2026, which has 1,794 entries. 334 of them carry an action date between October 2025 and September 2026.

A 37-page PDF with about 1,800 rows of provider actions.

Actions come from more than one authority, and the report keeps historical actions alongside current ones.

Reinstatement dates appear on the same report, so check every row for a name rather than stopping at the first.

Each figure counts entries in the current edition of the state’s official list by the action date the state publishes for each entry: the exclusion, suspension or termination date, depending on the list. Entries with no date are counted in the list size but not in any month.

A state list shows what the state publishes today. When a state removes an entry, for example after reinstatement, it leaves every month it was counted in, and a new edition can add an entry with an earlier date. The figures describe the edition named on the page.

States publish on their own schedules and some lists cover terminations or suspensions rather than exclusions, so counts are not comparable across states as rates. No names or identifiers are published here. To check a name, use the free exclusion check.
