About 173 action rows, plus alias rows that sit directly under the provider they belong to.

The workbook file name carries a date, so open it from the DPHHS page rather than a saved link.

DPHHS states the list is not all-inclusive and cannot by itself confirm enrollment status.

Each figure counts entries in the current edition of the state’s official list by the action date the state publishes for each entry: the exclusion, suspension or termination date, depending on the list. Entries with no date are counted in the list size but not in any month.

A state list shows what the state publishes today. When a state removes an entry, for example after reinstatement, it leaves every month it was counted in, and a new edition can add an entry with an earlier date. The figures describe the edition named on the page.

States publish on their own schedules and some lists cover terminations or suspensions rather than exclusions, so counts are not comparable across states as rates. No names or identifiers are published here. To check a name, use the free exclusion check.
