What is on the list
- Published by the Alabama Medicaid Agency’s fraud and abuse division, in both PDF and Excel.
- A suspension remains in effect until the agency removes the provider from the list; the passage of time alone does not end it.
- The Excel version is the easier one to search and to keep as evidence of what you checked.
How to check a name against it
- Download the current Excel file from the Suspended Providers page.
- Search by last name and by business name; providers may be listed under either.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
