NPPES and NPI: organization identity, scope and limitations

Use CMS provider identity data to verify organization names and NPIs, with dated source evidence and separate screening checks.

Organization registry coverage

The organization registry is disabled; live coverage is not established.

What the registry establishes

NPPES is the CMS National Plan and Provider Enumeration System. Its public data connects National Provider Identifiers with submitted provider information. CMS explicitly says that issuance of an NPI does not establish that a provider is licensed or credentialed. An NPI registry record does not establish Medicaid eligibility or absence of participation restrictions.

Monthly baseline and weekly updates

CMS publishes a full monthly downloadable file, weekly incremental files and a monthly deactivation report. Version1 support ended March3,2026; Version2 expands name field lengths. A weekly file contains changes since the previous weekly or monthly file and cannot replace a complete baseline by itself.

Organization records and identity verification

Exclia prioritizes Type2 organization records for identity enrichment. An exact NPI join identifies a registry record; a name-plus-state result remains an unverified identity lead. Check the underlying evidence and relevant screening sources separately. Additional organization names and practice information may require agency verification.

NPI deactivation and reactivation

An NPI deactivation is a registry event. It is not a federal OIG reinstatement or a determination of Medicaid participation. Registry updates preserve the published dates, and a later reactivation may restore the registry identifier without establishing licensure, credentials or program eligibility.

Coverage and publication limits

Registry metadata must show the activated organization version, original source checksum, retrieval anchor and data period. Disabled, stale or incomplete imports do not establish live coverage. NPPES does not count as a Medicaid exclusion database. A published guide or recorded fixture does not establish an active production registry.

Individual pages remain gated

Type1 individual ingestion and public person pages remain deferred pending legal review and a live dispute mechanism. This organization registry does not publish person pages. Organization pages also require review, provenance and quality sampling before indexable rollout.

Review coverage and screening sources

Use the coverage metadata and source-specific evidence to understand what a check covers. Registry identity and screening findings answer different questions.
