State Exclusion List
AHCCCS publishes an Excel list of the exclusion actions it has taken against providers and individuals in Arizona Medicaid under A.A.C. R9-22-1802, with each exclusion’s effective and end dates.

What is on the list
- Under Arizona law, items and services furnished, ordered or prescribed by a provider under an exclusion are not reimbursed by AHCCCS or its contractors.
- About 800 rows. Each row has the provider name, an NPI where AHCCCS publishes one, and the exclusion’s effective and end dates; a few have no fixed end date.
- AHCCCS withholds individuals’ identifiers, so many rows show a placeholder instead of an NPI. Identifying details may be available from AHCCCS on request.
- Reinstatement is not automatic: a provider applies after the end date, and a rescinded exclusion is removed from the list.
- This list is separate from AHCCCS’s list of active payment suspensions; check both.
How to check a name against it
- Download the current workbook from the AHCCCS State Exclusion List page.
- Search the provider name column, then compare the NPI where one is listed and the effective and end dates.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
A state list covers that state’s program. The federal LEIE and SAM.gov lists apply everywhere, so a complete check includes them.
Check names against every list at once
Exclia checks your employees, contractors and vendors against federal and state lists, keeps a dated record of every check, and flags potential matches for your review.
