What is on the list
- Each month DHCS publishes a new CSV with the month in its file name (for example, “august-2026”), so bookmark the catalog page rather than the file.
- About 23,400 rows. Each row has last, first and middle name, AKA/DBA names, addresses, provider type, license number, provider numbers and the date of suspension.
- The “Active Period” column reads “Indefinitely effective” for current suspensions. Reinstated providers are removed from the list rather than marked.
- The provider number field mixes Medi-Cal numbers, license numbers and NPIs, sometimes several in one cell — match on the NPI only when exactly one appears.
- Organizations are listed in the last-name column with no first name, so search that column for business names too.
How to check a name against it
- Download the latest CSV from the California open data catalog page for the S&I List.
- Search the last-name column for the surname or business name, then compare first name, AKA/DBA names and provider numbers.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
