What is on the list
- About 93 rows. Each row has the name, business, specialty, address, effective date, the period of the action (“Indefinite” or a number of years) and the action.
- A provider appears once per business they operated, so the same name can repeat with the same date.
- The list has no NPIs. Compare the name together with the business and address.
- The list does not say what happens when a time-limited action ends, so an older time-limited entry needs confirming with DSS.
How to check a name against it
- Open the Administrative Actions List on the DSS Quality Assurance page.
- Search the Name and Business columns for the person’s or organization’s name, then compare the address and specialty.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
A state list covers that state’s program. The federal LEIE and SAM.gov lists apply everywhere, so a complete check includes them.
Check names against every list at once
Exclia checks your employees, contractors and vendors against federal and state lists, keeps a dated record of every check, and flags potential matches for your review.
