What is on the list
- The download link is labelled XLS but serves a modern Excel (XLSX) workbook.
- About 1,375 rows with last, first and middle name, business name, provider type, state, sanction date and NPI.
- Individuals and businesses are in separate columns; search the business-name column for organizations.
- Some sanction dates are stored as Excel date numbers rather than text — sort and filter on the date column with that in mind.
- A few people appear on more than one row; check each row for a name.
How to check a name against it
- Download the current workbook from the DCH Office of Inspector General page.
- Filter by last name (or business name), then compare first name, NPI and state.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
A state list covers that state’s program. The federal LEIE and SAM.gov lists apply everywhere, so a complete check includes them.
