Hawaii Med-QUEST publishes a combined list of provider exclusions and reinstatements. An expired period alone does not mean a provider was reinstated.

What is on the list
- Exclusions and reinstatements are published together, so a provider may appear with both.
- Med-QUEST is explicit that an elapsed exclusion period is not, by itself, a reinstatement.
How to check a name against it
- Open the current list from the Med-QUEST provider page.
- Find every entry for the name and read the exclusion and any reinstatement entry together.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
