What is on the list
- Results include terminations, suspensions and voluntary withdrawals; read the action type on every result.
- A voluntary withdrawal is not an adverse finding and should be treated differently from a termination.
- Readmission or a discretionary removal takes a provider off the list, so confirm current status before acting.
How to check a name against it

- Search by name and note the action type and dates for each result.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
