What is on the list
- FSSA describes the termination list as informational, not an active exclusion list. A provider listed there may have re-enrolled since.
- About 110 rows. Each row has the provider name, the NPI, the service location city and state, and the termination effective date.
- Re-enrolled providers are on a separate PDF and can still appear on the termination workbook, so check both.
How to check a name against it
- Download the termination workbook and the re-enrollments PDF from the FSSA page.
- Search the workbook for the name and compare the NPI, then check whether that NPI appears on the re-enrollments list with a later date.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
A state list covers that state’s program. The federal LEIE and SAM.gov lists apply everywhere, so a complete check includes them.
Check names against every list at once
Exclia checks your employees, contractors and vendors against federal and state lists, keeps a dated record of every check, and flags potential matches for your review.
