Kentucky DMS publishes one list of providers whose agreements were terminated and providers under an exclusion. It is separate from the federal lists.

What is on the list
- Terminated agreements and exclusions appear on the same list; read the action for each entry.
- DMS treats the state list and the federal lists as separate checks — check both.
How to check a name against it
- Open the list from the DMS provider enrollment page.
- Search for the name and record the action and date.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
