What is on the list
- Each entry identifies the sanctioning authority, date and reason.
- When a sanction is removed, the provider comes off the list and payment eligibility resumes.
- The Excel version is the practical one for searching.
How to check a name against it
- Download the Excel list from the MDHHS provider sanctions page.
- Search by name and read the authority, date and reason.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
A state list covers that state’s program. The federal LEIE and SAM.gov lists apply everywhere, so a complete check includes them.
Check names against every list at once
Exclia checks your employees, contractors and vendors against federal and state lists, keeps a dated record of every check, and flags potential matches for your review.
