What is on the list
- Minnesota keeps individual and group providers on two separate lists — check both.
- Owner information appears only for enrolled individuals.
How to check a name against it
- Open the individual providers list from the MHCP provider manual.
- Search by name, then check the group list too.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
