MMAC Sanction List
Missouri MMAC publishes a workbook of terminated Medicaid providers with each termination date, provider type and the reason for the termination.
At a glance
- Published by
- Missouri Medicaid Audit and Compliance (MMAC)

- Monthly, per the publisher (dated in the file name)
- Covers
- Medicaid provider terminations
What is on the list
- Reasons range from fraud, convictions and license revocations to bad debt and administrative non-compliance, and each row states its own.
- About 280 rows. A provider appears once per enrolled provider type, so the same name and NPI can repeat.
- Each edition has its date in the file name, so open the file from the MMAC page rather than a saved link.
How to check a name against it
- Download the current sanction list from the MMAC Provider Sanctions page.
- Search the provider name column, then compare the NPI and license number.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
A state list covers that state’s program. The federal LEIE and SAM.gov lists apply everywhere, so a complete check includes them.
Check names against every list at once
Exclia checks your employees, contractors and vendors against federal and state lists, keeps a dated record of every check, and flags potential matches for your review.
