What is on the list
- ODM includes the provider’s last known address and date of birth to help confirm identity.
- The list does not include individuals terminated from Ohio Medicaid because of license issues.
- ODM updates the file frequently and asks users to download the current one each time.
- A voluntary termination is different from an adverse action and has different consequences.
How to check a name against it
- Download the current file from the ODM provider exclusion and suspension page.
- Search by name and compare address and date of birth.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
