What is on the list
- Three entries as of October 2026.
- Each row has the provider name, NPI, duration and effective date. Every current duration reads “Indefinite”.
- People and organizations share one name field, so search the full name and compare the NPI.
- The page has no reinstatement column; a provider OHA removes from the table is no longer listed.
How to check a name against it
- Open the Sanctioned Providers section of the OHP provider enrollment page.
- Search the table by name or NPI.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
A state list covers that state’s program. The federal LEIE and SAM.gov lists apply everywhere, so a complete check includes them.
Check names against every list at once
Exclia checks your employees, contractors and vendors against federal and state lists, keeps a dated record of every check, and flags potential matches for your review.
