What is on the list
- One workbook, titled with the month (for example, “September 2026”), at a stable “most current list” link on the provider portal.
- Each row has the Vermont Medicaid provider ID, NPI, full name, provider type, city, state, reason, effective date and any reinstatement date.
- The workbook’s key: a reinstatement date means the period ended and the provider was reinstated; no date means the action remains in effect.
- The end-date column holds a far-future placeholder rather than a real end date.
- Names are a single field and some organizations use commas too, so search the full name.
How to check a name against it
- Open the current workbook from the provider exclusions page on vtmedicaid.com.
- Search the full name, then check the reinstatement column.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
Check the federal lists too
