What is on the list
- Each entry has a name, location address, DSHS license field, action date and ProviderOne ID.
- It is independent of the HCA Apple Health list; check both.
How to check a name against it
- Download the DSHS workbook from the HCA provider termination and exclusion page.
- Search by name and compare the ProviderOne ID.
- Treat a name match as a potential match until the identity is confirmed with the publisher, using identifiers such as NPI, license number, date of birth or address.
