Exclia

Indiana Medicaid provider terminations by month

Indiana FSSA publishes a workbook of providers whose Medicaid and CHIP participation was terminated for cause, and a separate list of providers who later re-enrolled.

Built from the Provider Terminations edition Exclia retrieved on October 15, 2026, which has 112 entries. 3 of them carry an action date between October 2025 and September 2026.

FSSA describes the termination list as informational, not an active exclusion list. A provider listed there may have re-enrolled since.

About 110 rows. Each row has the provider name, the NPI, the service location city and state, and the termination effective date.

Re-enrolled providers are on a separate PDF and can still appear on the termination workbook, so check both.

Each figure counts entries in the current edition of the state’s official list by the action date the state publishes for each entry: the exclusion, suspension or termination date, depending on the list. Entries with no date are counted in the list size but not in any month.

A state list shows what the state publishes today. When a state removes an entry, for example after reinstatement, it leaves every month it was counted in, and a new edition can add an entry with an earlier date. The figures describe the edition named on the page.

States publish on their own schedules and some lists cover terminations or suspensions rather than exclusions, so counts are not comparable across states as rates. No names or identifiers are published here. To check a name, use the free exclusion check.
