Reasons range from fraud, convictions and license revocations to bad debt and administrative non-compliance, and each row states its own.

About 280 rows. A provider appears once per enrolled provider type, so the same name and NPI can repeat.

Each edition has its date in the file name, so open the file from the MMAC page rather than a saved link.

Each figure counts entries in the current edition of the state’s official list by the action date the state publishes for each entry: the exclusion, suspension or termination date, depending on the list. Entries with no date are counted in the list size but not in any month.

A state list shows what the state publishes today. When a state removes an entry, for example after reinstatement, it leaves every month it was counted in, and a new edition can add an entry with an earlier date. The figures describe the edition named on the page.

States publish on their own schedules and some lists cover terminations or suspensions rather than exclusions, so counts are not comparable across states as rates. No names or identifiers are published here. To check a name, use the free exclusion check.
