About 9,117 rows with provider name, license number, NPI, provider type and exclusion effective date.
The provider name is a single field and does not say whether it is a person or an organization, so search the full name either way.
Some names include nicknames in quotation marks, and a few exact duplicate rows exist in the source.
Each figure counts entries in the current edition of the state’s official list by the action date the state publishes for each entry: the exclusion, suspension or termination date, depending on the list. Entries with no date are counted in the list size but not in any month.
A state list shows what the state publishes today. When a state removes an entry, for example after reinstatement, it leaves every month it was counted in, and a new edition can add an entry with an earlier date. The figures describe the edition named on the page.
States publish on their own schedules and some lists cover terminations or suspensions rather than exclusions, so counts are not comparable across states as rates. No names or identifiers are published here. To check a name, use the free exclusion check.
