ODM includes the provider’s last known address and date of birth to help confirm identity.
The list does not include individuals terminated from Ohio Medicaid because of license issues.
ODM updates the file frequently and asks users to download the current one each time.
A voluntary termination is different from an adverse action and has different consequences.
Each figure counts entries in the current edition of the state’s official list by the action date the state publishes for each entry: the exclusion, suspension or termination date, depending on the list. Entries with no date are counted in the list size but not in any month.
A state list shows what the state publishes today. When a state removes an entry, for example after reinstatement, it leaves every month it was counted in, and a new edition can add an entry with an earlier date. The figures describe the edition named on the page.
States publish on their own schedules and some lists cover terminations or suspensions rather than exclusions, so counts are not comparable across states as rates. No names or identifiers are published here. To check a name, use the free exclusion check.
